A vent valve broke off the top of an elbow due to fatigue stress. This left an open hole in the system and resulted in a major steam leak. We engineered a plug that was inserted into the open hole to safely seal the leak. The addition of Belleville Spring Washers ensured that a constant load would remain on the plug in the event of thermal expansion / contraction of the piping.
